Webinar: Primary care models for tackling multi-morbidity

On 17 November Peter Groenewegen will present a webinar, titled Primary Care Models for Tackling Multimorbidity.

 In this webinar Peter will address the problem of fragmentation of health care. He will present information on programmes in EU countries to integrate care for patients with multiple chronic diseases, based on information collected in an European study – the ICARE4EU study. The potential for improvement according to patients in the important areas of continuity, comprehensiveness and patient involvement will be presented on the basis of the QUALICOPC study. Finally, Peter will touch upon the issue of the transferability of practices from one country to another.

Peter is member of the Executive Board of EFPC, senior researcher at NIVEL and professor at Utrecht University. The webinar is based on his contribution to the Baltic Policy Dialogue on ‘Strengthening primary care: implementing change towards better coordination and more multi-professional working’, organized by the European Observatory on Health Systems and Policies earlier in November in Tallinn (Estonia)
